Community Needs Assessment

T.J. Regional Health continues to research the most effective ways to best serve the needs of our communities. Over the past several years, the organization has participated in conducting a Community Needs Assessment in partnership with the Barren River Community Health Planning Council. The Community Health Needs Assessment determined the areas of greatest health need for the region. Below are links to the Community Health Needs Assessment and the Executive Summary published by the Barren River Community Health Planning Council. Also available is T.J. Samson Community Hospital's Community Health Needs Assessment and Strategic Implementation Plan, which demonstrates how the hospital plans to help meet the needs identified in the community assessment.

Additionally, the organization has participated in conducting a Community Health Improvement Plan in partnership with the Lake Cumberland District Health Department/Adair County W.A.T.C.H (Working Actively Toward Community Health) Coalition. The Lake Cumberland Community Health Improvement Plan is a result of the MAPP process (Mobilizing Action through Partnership and Planning) and determines the areas of greatest health need for each county in the Lake Cumberland District. The plans are driven by all the community health coalition partners to help improve the health status of a county within the region. Health Columbia's Community Health Needs Assessment and Strategic Implementation Plan, that demonstrates how the hospital plans to help meet the needs identified within the community assessment.

The conducting of the Community Health Needs Assessments and the implementation of the Strategic Plans are both in an effort to comply with the Affordable Care Act and to maintain organizational nonprofit status.
